Vaccination Certificate

Would you like proof of vaccination for travel or for entry to an event or indoor location?

You can visit the website CoronaCheck.nl or the app CoronaCheck on your smartphone to receive a vaccination certificate.

Have you tried CoronaCheck.nl and couldn’t the site find your vaccination certificate?

Please call the Coronacheck helpdesk first: 0800-1421

Only in the case that you have been fully vaccinated by our GP practice we might be able to help you.

Send an e-mail to info@huisartsenkastelenbuurt.nl, indicating that you want to receive proof of complete vaccination, because this was not successful via CoronaCheck.nl. Also include your contact details.

If it is possible for us to provide you with a vaccination certificate, we will send it to you.

There is no point in calling our practice about this.

Check the government website for more information about what you can do yourself to obtain a vaccination certificate.
